Hi!  Just a few simple intake steps to get you started on your path to wellness at CIP ...

1.  Please fill out our New Client Intake Forms and submit via email to or bring them with you to your first appointment.  Just a basic health intake form, liability agreement and publicity release.  We will discuss all physical concerns and injuries more in depth, as well as your personal goals, at your first visit! Please access forms here.

2.  Go to and set up your personal profile. This will enable you to see your future/past schedule, cancel appointments as necessary and keep billing information current.

3.  That's it!
